This form is for use to notify the Commission of changes you are making to your governing document including your charity’s objects or its name.

The form provides you with a series of options and by selecting those relevant to your charity and your circumstances you will be able to determine if the Commission needs to provide approval to the changes you are seeking.

When you are in the form please do not use the BACK button on your browser as you will lose any information you have input.

At certain points you may receive a suggestion to open a separate window. Your form will still be available to you when you leave this window.

You cannot use this form if your charity is governed by a Royal Charter. Please refer to our web guidance for Royal Charter Charities.

At the end of the form you will have the option of providing us with feedback on how useful you found it. This feedback is very important to us and will ensure that we can continue to refine the form so it remains of real use to charities and their advisors.
