The information requested in this form will enable us to vest charity land in the Official Custodian for Charities under sections 69-72 and 90 of the Charities Act 2011.

The form can be completed by any person having a competent knowledge of the facts and acting on behalf of all the trustees. If the person completing the form is not known to us as a correspondent or trustee of the charity we may need to ask them to verify their identity.

Anyone who knowingly or recklessly provides false or misleading information on this form will be guilty of an offence under section 60(1)(b) of the Charities Act 2011.
