The information requested in this form will enable us to remove your charity from the Register of Charities. We aim to remove your charity within 15 working days of receipt of the form and you should check the online register for confirmation of removal.

The form can be completed by any person having a competent knowledge of the facts and acting on behalf of all the trustees. If the person completing the form is not known to us as a correspondent or trustee of the charity we may need to ask them to verify their identity.

Anyone who knowingly or recklessly provides false or misleading information on this form will be guilty of an offence under section 60(1)(b) of the Charities Act 2011.
